SMS Intake Consent — Patient Onboarding
Public compliance view of the Alto Practice patient intake steps where mobile numbers are collected and SMS consent is presented. No login required.
During secure electronic onboarding, patients enter their mobile number and review SMS consent on the same step. The consent checkbox defaults to unchecked. SMS is optional and is not required to receive healthcare services.
For carrier and compliance review: Alto Practice collects a patient's mobile number in up to two separate steps. SMS opt-in always requires a separate, affirmative action on the same screen as the phone field. The SMS consent checkbox defaults to unchecked.
- Step 1 — Staff Add Patient form (screenshot): practice staff enter the patient's mobile number when creating a new patient / onboarding invitation. SMS defaults off.
- Step 2 — Patient electronic registration (screenshot): when the patient opens their secure onboarding link, they confirm or enter their phone on the profile step and may opt in to SMS on that same step via an unchecked consent checkbox.
Step 1
Staff Add Patient form — where the phone number is first collected
In the app: Patients → New Patient (New Patient Onboarding dialog)
When practice staff add a new patient or send an onboarding invitation, they enter the patient's mobile number in the Phone Number field on this form. The SMS / Text toggle on the same screen defaults to off. Once a phone number is entered, the SMS disclosure appears on the same screen — it identifies Alto Practice as the messaging platform sender. Staff enable SMS only after reading the disclosure and documenting consent.
- Phone field: labeled Phone Number * — this is where staff enter the patient's mobile number.
- SMS toggle: optional SMS / Text toggle defaults off on the same form.
- SMS disclosure: shown on the same screen once a phone number is entered; begins with "Alto Practice may send appointment reminders…"
- Not required for care: patients can be invited without SMS enabled.
Step 2
Patient electronic registration — phone entry and SMS consent (same step)
In the app: secure link /onboarding/{token} → Complete your profile step
When the patient opens their secure onboarding link, they reach the Complete your profile step for contact information. On this step they confirm or re-enter their mobile number in the Phone field. If a number was already provided by staff in Step 1, it may appear pre-filled; the patient can update it here.
On this same step, directly below the phone field, the patient sees the full SMS disclosure, links to the Privacy Policy, SMS Terms & Conditions, and Terms of Service, and an unchecked consent checkbox. The patient must check the box to opt in to SMS.

- SMS consent: Text message notifications (optional) section on the same step, with disclosure text and an unchecked consent checkbox.
- Separate from other intake: SMS consent is not bundled with general intake terms; it is its own optional agreement.
